Common causes
- One key reused for two different requests.
- A retry that changed the body.

This Idempotency-Key was already used with a different payload. Use a new key, or resend the original payload.
Example
The body arrives as application/problem+json. Most responses add detail about the occurrence, and some add fields of their own.
